A: DENTAL-Guard is a discount dental plan that uses the nations largest network of dentists. All network dental offices have agreed to use these discounted fees as payment in full, creating amazing savings for our plan members. This program is NOT insurance.

Dental Specialties

Endodontist (EN)
Treats problems with the nerve of the tooth. Performs root canal treatments, root amputation (removing a root on a multi-rooted tooth) and apicoectomy (surgically removing the tip of the root).

General Dentist (GP)
General family dentist without additional specialty training.

Oral Surgeon (OS )
Performs surgery in the head area, including: simple and complex extractions involving removal of soft tissue or overlying bone or remaining roots, impacted teeth (especially wisdom teeth), soft tissue biopsies, removal of tumors, implant placement, complex jaw realignment surgeries (orthognathic surgery) involving facial or bite discrepancies, jaw or facial fracture stabilization (repair of fractured cheek or jaw bones) and repair of soft tissue problems (cleft palate or lip).

Orthodontist (OR )
Straightens patient's teeth through movement of the teeth through bone by the use of bands, wires, elastics, headgears and other fixed or removable appliances or retainers.

Pedodontist (PD)
Focuses on younger patients from children as young as one or two to early adulthood. Can detect, treat, or refer problems with decayed, missing, crowded, or crooked teeth.

Periodontist (PE )
Deals with soft tissue of the mouth and the supporting structures (bone) of the teeth. Treats gingivitis (inflammation of the gingival tissue) as well as periodontitis (gingival tissue and bone disease). Performs simple and deep pocket cleanings, root planning, crown lengthening, soft tissue and/or bone grafting, gingival or flap procedures, soft tissue recontouring or removal (gingivoplasty or gingivectomy), hard tissue recontouring (osteoplasty) and implant placement.

Prosthodontist (PR )
Deals with simple to complicated full-mouth restorations or full-mouth rehabilitation. Performs crowns, fixed bridges, partial dentures, full dentures, overdentures, implant reconstruction, full mouth reconstruction/occlusal rehabilitation, cosmetic dentistry, treatment of temporomandibular /orofacial pain disorders.

Q: How do I use the DENTAL-Guard?

A: As soon as we receive your application and payment you will be eligible to use the plan. We will provide you with a DENTAL-Guard Dental identification card. Simply present your identification card to any DENTAL-Guard providers when services are performed and make payment directly to the provider.

Q: How often can I use the Plan?

A: DENTAL-Guard places your dental care first. There are no limits on how much you can use your dental plan or how many times you can visit your provider

Q: When can I start receiving discounts on dental work?

A: Your DENTAL-Guard discount dental plan can be utilized the day of activation. Just show the participating dental provider your Member ID Card and they will honor the discounted rate according to the fee schedule of your plan.

Q: Do I receive discounts if I see an out of network dentist?

A: Plan members will not receive discounts on dental care when visiting an out-of-network provider. Members must always see a participating dentist to receive the discounts associated with their discount dental plan. However, if a dental care provider would like to join a network, they may contact our customer service center.

Q: Who is eligible to join DENTAL-Guard?

A: DENTAL-Guard is available for anyone to join. We offer individual dental plans, family dental plans and group dental plans. We also offer group discounts for 5 or more.

Network Access

Your Dental Guard discount plan has dentists in 48 states and the District of Columbia.


Dental Guard is NOT INSURANCE, it is a membership in a Discount Dental Plan. This Membership Agreement ("Agreement") is entered into by and between Family Medical Plans,Inc , a Arizona Corporation ("FMP"), and the individual whose name and address are herein set forth ("Member"), on the date indicated next to Member's signature with privileges effective as of the date payment of the membership fee is received by FMP.

The Plan's Providers and Services are set forth in the New Member Packet, containing info which is also available on-line at www.Discounted-Dental.com  Member understands and agrees that the Providers and Services may change from time to time.

The term of this Agreement is for one (1) year from the Effective Date, and will be automatically renewed for additional one year terms unless either party sends a written termination that is received by the other party at least 10 days before the end of the current term. The Member may rescind this Agreement by sending written notice to FMP within 30 days of signing for a full refund, minus a nominal $5 processing fee. Member understands and agrees that after the 30-day rescission period or after any use of the Providers and Services, the membership fee becomes non-refundable. If Member has elected to pay in monthly installments, an early termination fee in the amount of the Member's annual membership fee (less monthly installments already paid) shall become due in full if the Member chooses to terminate the membership for any reason before the end of the term.
